All appointments are scheduled for 55 minutes

Initial Physical Therapy Evaluation – $125
Follow-up sessions – $110

There are times when an abbreviated session is warranted.  If so, these 25-30 minute appointments will be scheduled separately vs. through the website.

All payments are made via cash, check, credit/debit card, or Venmo. If applicable, you may also use your Flexible Spending Account (FSA) or Health Savings Account (HSA) dollars on physical therapy.

WHAT TO EXPECT

Please take the time to fill out all of the forms ahead of time, so your evaluation can begin with a conversation about what brought you to physical therapy, and what you hope to accomplish.

Please come in athletic clothing or loose fitting clothing so you may actively move around and be able to exercise.

The appointment is up to 55 minutes long and will include:

  • addressing paperwork and payment policy
  • performance of a physical therapy evaluation
  • discussion of findings
  • prescription of exercises
  • practice of exercises
  • any other treatments ie. functional dry needling, manual therapy
  • discussion of plan of care
  • scheduling future visits

FEE FOR SERVICE

Crossroads Physical Therapy is a fee-for-service practice. This means we do not bill your insurance. If requested, you will be provided with a Superbill that you can submit to your insurance for any available out-of-network benefits. Please see the “List of Questions” you will need to ask of your insurance company to see how they might reimburse you for out-of-network services.

DO I NEED A PHYSICIAN REFERRAL FOR PHYSICAL THERAPY?

Colorado is a direct access state meaning you do not need a referral from a physician to see a physical therapist.

WHAT IF I HAVE MEDICARE?

Crossroads Physical Therapy is not a Medicare provider and therefore, due to Medicare rules, cannot accept cash payment for physical therapy services from Medicare beneficiaries at this time.
